Why Pereste exists

The patients health systems most struggle to reach — rural, older, lower-literacy — are also the patients most likely to be readmitted, to fall out of care plans, and to be underserved by digital tools built for someone else.

We started Pereste to build clinical AI for those patients specifically: models that adapt to the person in front of them, and deployments that respect the institutions already trying to help.

What we do

Two things, tightly coupled: the model, and the implementation.

Model

Clinical AI built for the people on the receiving end.

We develop the underlying AI — adaptive to each patient, grounded in clinical context, and trained to behave well when the stakes are real. Our work is protected by pending intellectual property.

Implementation

Deployed inside the institutions we partner with.

Our solutions run where patient data already lives, under the governance the institution already trusts. No PHI egress, no parallel infrastructure to maintain.

Where we're going

Our first solution, HearthAI, focuses on diabetic patients at hospital discharge. The same underlying approach extends to other chronic conditions where adherence and understanding drive outcomes — the same patients, the same gap, applied to new ground.

Pereste is the company that holds that IP and brings each solution to the institutions ready to deploy it.
